Please aim to arrive 20-30 minutes BEFORE scheduled appointment: time is needed for registration; complete questionnaires (if needed); download monitoring devices; point of care testing as needed, rooming and vital sign checks

Minimum information needed: allergies/adverse drug reactions, medications, pharmacy information, contact information, referring physician and members of health care team

Please request all relevant records PRIOR to the visit (laboratory tests, imaging studies, office notes, pathology reports)

Please complete patient questionnaire (will help maximize efficiency of the visit)

Please sign up for Epic patient portal MyChart:
Test results, visit summaries will be posted, used for refill requests, non-urgent and secure communication.

Return patients:
Please aim to arrive 10-20 minutes before appointment time (helps ensure visit starts on time)
